GRADE 6 IMMUNIZATION REQUIREMENTS
All students entering grade 6 during the 2012-13 school year must have a booster immunization containing diphtheria and tetanus toxoids, and an acellular pertussis vaccine—commonly known as Tdap. Obtaining this immunizations is the parent’s responsibility, and it is readily available at all physician’s offices, as well as Lewis County Public Health (phone 376-5453).
